Florida Psychiatric Supervision Agreement and Protocol — Sample

1. Scope and clinical protocols

The parties will define covered psychiatric services, patient ages, practice settings, treatment protocols and medication limits in a signed schedule. Duties must remain within verified licensure, training and competence. Prescribing eligibility, including any controlled-substance authority, must be verified separately; this sample grants no authority.

2. Consultation and coverage

The physician will provide timely consultation during covered clinical activities through agreed secure channels. The completed protocol will identify alternate coverage, referral arrangements and conditions requiring physician evaluation. Emergencies require immediate appropriate care, without waiting for a supervision meeting or message reply.

3. Meetings and record review

The proposed Euthymic service includes a documented 60-minute meeting each month, with additional consultation as clinically needed. This is a service term, not a statement of a universal statutory meeting requirement.

The signed protocol will specify the record-review method and frequency, case selection, reviewer and follow-up, considering patient volume, clinical risk and APRN experience. This sample sets no blanket chart percentage. Any additional binding practice, facility, payer or legal requirements must be addressed.

4. Records and confidentiality

Provide a separate authorized EHR reviewer account or sufficient clinical records through an approved secure channel. Do not share passwords. Each review must be traceable to the source chart. De-identification, lawful disclosure, access controls and retention arrangements must be addressed; a software BAA alone does not establish compliance.

5. Fee

The proposed direct-pay fee is $600 per month, due on the 15th, starting on the agreed effective date. Identify the payer, payee and any partial-month terms in the final agreement. Prior balances and separate payment arrangements must be reconciled individually.

6. Completion, review and termination

Before signing, complete the provider-specific scope, formulary, coverage, record-review and practice-location schedules, verify credentials and arrange applicable notices. The final agreement must specify review, termination and safe patient-transition procedures. Existing obligations remain in place until a properly executed revision replaces them.
